On the basis of the new version of Law no. 101/2000 Sb. (Law on the Protection of Personal Information) it is necessary to have your consent in processing personal information for genetic testing. Without this consent THE CONDUCTING OF GENETIC TESTS IS NOT PERMITTED WITHIN THE CZECH REPUBLIC.

The Trustee of Personal Information: Genomac International, s.r.o. residing at Hostalkova 42, Prague 6, Postal Code 169 00; ID No: 26475821; Tax ID No: CZ26475821. In accordance with Law No. 101/2000 Sb. on the Protection of Personal Information (hereafter “Law”), the Subject grants the Trustee the right to collect, process, store, and use the Subject’s personal information. Processed sensitive personal information includes in particular genetic information obtained from DNA samples provided by the Subject, wherein processed personal information is communicative contact information for the sending of results. The aim of processing is the analysis of deoxyribonucleic acids (hereafter “DNA”) in paternity determination, communication with the user on all the Trustee’s services, and the fulfillment of items commissioned. Provided information may be accessible only to those permitted by the Subject in compliance with respective legal appointment (i.e. active entities in criminal procedure, etc.). The Subject grant

s their agreement to processing for an indefinite time period. The Subject is entitled by § 12 of the Law to solicit information from the Trustee, regarding what personal information the Trustee is processing on the Subject. The Trustee has the right, in providing such information, to claim an appropriate fee not exceeding the necessary expense of providing the information. Providing the personal information is voluntary and the Subject is entitled to withdraw it at any time. In such a case, the Trustee will carry out the liquidation of this personal information.
